Case Vignette #1-Submitted September 23, 2009

Submitted by: Linda B Cottler, PhD, MPH and Catherine Striley, PhD, ACSW, MPE

} Tina presents to Doctor F for labored breathing after her mental health agency case manager called and made the appointment. Doctor F is currently enrolling for a study of untreated asthma. Tina appears well-groomed and smiles at the receptionist.

} Should Dr. F’s coordinator screen Tina for the study? Why or why not?

} How confident are you:

That Tina would participate in the study?

That Tina would attend every study visit?

Case Vignette #2

Date: December 21, 2009

Comment: For perinatal depression, exclusion criteria is active substance abuse; difficult to treat depression when substance abuse is ongoing; her group usually recommends the person get treatment for substance abuse and then will accept people with a history of substance abuse.

Case Vignette #3

Date: December 21, 2009

Name: Dr Marie Claire Van Hout
Country: Ireland
Comment: From my experience, ethical concerns relating to research participant comprehension and decision making abilities remain present, in terms of potential participant intoxication, withdrawal and addiction clouding the informed consent process. Other special concerns remain with regard to drugs research conducted with minors, particularly in terms of obtaining adequate informed consent, and also possible child protection guidelines.

Case Vignette #4

Date: December 21, 2009

Name: Javier Gontier Bertiola

Country: Chile

Comment: My comment:
In Chile there is little research about drugs and clinical trials. Most of the data and information we have come from other countries (US). Clinicians use medications to treat patients based on foreign scientific literature. Ethical issues in Chile have to do with how well the patients are informed about side effects and participation in research. For instance, how I do research with teenagers who do drugs (cocaine base paste) that are hospitalized for detoxification but have no adult responsible for them? My choice would be to ask for consent with the patient and the medical staff.
